3. No Doctor-Patient Relationship
Viewing this Site, reading its content, or submitting a contact or appointment request form does not create a doctor-patient relationship between you and our practice or any of its providers. A doctor-patient relationship is established only after you have been examined in person and we have expressly agreed to accept you as a patient.
4. Emergencies
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use this Site, our forms, or email to communicate about an urgent or emergency condition. We do not monitor Site submissions continuously and cannot respond to emergencies.
5. Appointment Requests
Our online form allows you to request an appointment. A submitted request is not a confirmed appointment. An appointment is scheduled only when a member of our staff contacts you and confirms a specific date and time.
Do not include detailed medical information, symptoms, diagnoses, treatment history, insurance identifiers, or other sensitive information in the form. Submissions are transmitted over the internet and are not a secure channel for health information.
6. Communications Consent
By providing your phone number or email address through the Site, you consent to be contacted by our practice at that number or address regarding your inquiry or appointment, including by phone call, text message, and email. Message and data rates may apply. Reply STOP to any text message to opt out of further text messages. Consent to receive marketing communications is not a condition of receiving care. Carriers are not liable for delayed or undelivered messages.

OUR TOTAL LIABILITY FOR ANY CLAIM ARISING OUT OF OR RELATING TO THE SITE OR THESE TERMS SHALL NOT EXCEED ONE HUNDRED DOLLARS ($100.00).

16. Governing Law and Venue
These Terms are governed by the laws of the State of Idaho, without regard to its conflict of laws principles. You agree that any action arising out of or relating to these Terms or the Site shall be brought exclusively in the state or federal courts located in Canyon County, Idaho, and you consent to the personal jurisdiction of those courts.
17. Severability
If any provision of these Terms is held invalid or unenforceable, that provision shall be enforced to the maximum extent permissible and the remaining provisions shall remain in full force and effect.
18. Changes to These Terms
We may revise these Terms at any time by posting an updated version on the Site and revising the "Last Updated" date. Changes take effect when posted. Your continued use of the Site after changes are posted constitutes your acceptance of the revised Terms.
19. Entire Agreement
These Terms, together with our Privacy Policy, constitute the entire agreement between you and us regarding your use of the Site and supersede any prior agreements on that subject.
