This Privacy Policy describes how Mitchell Chiropractic ("we," "us," or "our") collects, uses, and discloses information when you visit www.mitchell-chiropractic.com (the "Site") or submit an appointment request through it.

2. Important Notice About Health Information

This Privacy Policy governs the Site only. It does not govern the medical records we maintain about our patients.

If you are a patient of our practice, information we collect and maintain in connection with your care and treatment — including your medical records and billing information — is protected health information governed by our Notice of Privacy Practices and applicable federal and state law, not by this Privacy Policy. Please do not include detailed medical information, symptoms, diagnoses, or treatment history in any form, email, or message submitted through this Site. The Site's contact and appointment request forms are not a secure channel for health information. Use them only to give us your name and contact information and to tell us that you would like us to reach out. We will collect any clinical information we need through a secure channel or in person.

3. Information We Collect

3.1 Information You Provide

When you complete our contact or appointment request form, we collect:

Your name

Your phone number

Your email address

Any message or free-text you choose to include

Your preferred appointment day or time, if requested

We collect this information only when you voluntarily provide it. All fields marked as required must be completed in order to submit the form.

3.2 Information Collected Automatically

When you visit the Site, certain information is collected automatically by us and by third parties acting on our behalf:

IP address and approximate geographic location derived from it

Browser type, operating system, and device type

Pages viewed, links clicked, and time spent on pages

The website or search engine that referred you to the Site

Date and time of your visit

5. How We Share Information

We do not sell your information for money. We share information in the following circumstances:

Service providers. We use vendors to host the Site, deliver form submissions to us, manage our scheduling and communications, and provide analytics and marketing services. These vendors may access your information only to perform services for us.

Advertising and analytics partners. Through the analytics and advertising technologies described in Section 3.3, information about your visit — including a unique identifier, the pages you viewed, and actions you took on the Site — is transmitted to Google and Meta Platforms, Inc. These companies may combine that information with data they already hold about you and may use it for their own purposes, including improving their advertising products. Under some state privacy laws, this transfer is considered a "sale" or "sharing" of personal information for targeted advertising purposes. See Section 8 for how to opt out.

Mobile information. Mobile phone numbers and text message opt-in data are subject to the additional restrictions described in Section 6.7 and are never shared with third parties or affiliates for marketing or promotional purposes.

Legal and safety. We may disclose information when we believe in good faith that disclosure is required by law, subpoena, court order, or other legal process, or is necessary to protect the rights, property, or safety of our practice, our patients, or others.

Business transfers. If our practice is sold, merged, or reorganized, information may be transferred as part of that transaction, subject to applicable law governing patient records.

6. Text Message (SMS) Program

6.1 About the Program

Mitchell Chiropractic operates a text messaging program used to respond to appointment requests, coordinate and confirm appointments, send appointment reminders, and — for those who separately opt in — send occasional practice news and offers.

6.2 How You Opt In

We send text messages only to individuals who have given prior express consent. You may opt in by:

Checking the consent box on our appointment request form and providing your mobile number

Providing your mobile number and agreeing to text communications in person or by phone with our staff

Texting us first at 208-417-7919, or replying to a message from us

Consent to receive text messages is not a condition of receiving care or of purchasing any goods or services from us. You may decline text messages and still schedule and receive treatment.

If you separately opt in to promotional messages, that is a distinct consent from appointment-related messages, and you may withdraw either one independently.

6.3 Message Frequency

Message frequency varies. Appointment-related messages are sent in response to your requests and in connection with scheduled visits — typically no more than a few messages per appointment. If you opt in to promotional messages, you can expect no more than [4] messages per month.

6.4 Cost

Message and data rates may apply. We do not charge for text messages, but your mobile carrier's standard messaging and data rates apply to messages you send and receive. Contact your carrier for details about your plan.

6.5 Opting Out and Getting Help

Reply STOP to any message to opt out. You will receive a single confirmation that you have been unsubscribed, after which we will send no further texts to that number unless you opt in again.

6.6 Carriers and Delivery

Mobile carriers are not liable for delayed or undelivered messages. Message delivery is subject to effective transmission by your carrier and is not guaranteed. Supported carriers may change without notice.

6.7 How We Handle Your Mobile Information

No mobile information will be shared with third parties/affiliates for marketing/promotional purposes. Information sharing to subcontractors in support services, such as customer service, is permitted.

This means we do not sell, rent, or otherwise disclose your mobile phone number or your text message opt-in status to any third party for that party's own marketing or promotional use, and we do not include mobile numbers or opt-in data in any information shared with advertising or analytics partners. We disclose mobile information only to vendors that provide services to us — such as our messaging platform, scheduling system, and communications provider — and only so that they can deliver those services on our behalf. We may also disclose mobile information where required by law, as described in Section 5.

7. Retention

We retain contact and appointment request information for as long as reasonably necessary to respond to your inquiry and for our recordkeeping and legal compliance needs. If you become a patient, information relating to your care is retained in accordance with applicable medical records retention requirements. Automatically collected Site data is retained according to the default retention settings of the analytics and advertising platforms we use.

8. Your Choices

Opting out of advertising and analytics. You can:

Adjust your browser settings to block or delete cookies

Use a browser or extension that transmits a Global Privacy Control (GPC) signal, which we treat as a valid opt-out request where required by law

Install the Google Analytics Opt-out Browser Add-on at tools.google.com/dlpage/gaoptout

Adjust your Google advertising settings at myadcenter.google.com

Adjust your Meta advertising settings at facebook.com/adpreferences

Email and text messages. If you receive emails from us, you may unsubscribe using the link in the message. If you receive text messages from us, you may reply STOP to opt out; see Section 6 for full details of our text messaging program. Note that opting out of promotional messages does not by itself stop appointment-related communications, and vice versa — these are separate consents you may withdraw independently.

Access, correction, and deletion. Depending on where you live, you may have the right to request access to, correction of, or deletion of personal information we hold about you, to request a copy of that information in a portable format, to opt out of targeted advertising, and to be free from discrimination for exercising these rights. To make a request, contact us using the information in Section 13. We will verify your identity before responding. Requests relating to medical records are handled under our Notice of Privacy Practices rather than this policy.

Residents of other states. Idaho has not enacted a comprehensive consumer privacy statute. Several other states — including California, Colorado, Connecticut, Virginia, Texas, and others — grant their residents privacy rights of the kind described above. Whether those laws apply to our practice depends on statutory thresholds and exemptions. Regardless of whether we are legally required to do so, we will consider and respond to any request described in this section that we receive from a Site visitor.

9. Security

We use reasonable administrative, technical, and physical safeguards designed to protect information submitted through the Site. However, no method of transmission over the internet and no method of electronic storage is completely secure. We cannot guarantee the security of information you transmit to us, and you do so at your own risk.

10. Children's Privacy

The Site is not directed to children under 13, and we do not knowingly collect personal information from children under 13 through the Site. If you are under 18, please do not submit information through the Site; ask a parent or guardian to contact us on your behalf. If we learn that we have collected information from a child under 13 through the Site, we will delete it.

11. Third-Party Websites

The Site may contain links to third-party websites, including review platforms, social media, and patient portals operated by others. We are not responsible for the privacy practices or content of those sites. Review their privacy policies before providing information.

12. Changes to This Policy

We may update this Privacy Policy from time to time. When we do, we will revise the "Last Updated" date above. Material changes will be indicated by a notice on the Site. Your continued use of the Site after changes take effect constitutes acceptance of the revised policy.
